Application Process

• Request or print off an application packet from the Center for Disability Services. A list of local licensed diagnostic service providers may be obtained online or through CDS (843-953-1431).
• Submit a completed SNAP application form that is signed and dated and two copies of a psychoeducational assessment or medical documentation to SNAP Services. Reports must meet the criteria required by the College of Charleston.
  Criteria for Documentation of a Learning Disability, Criteria for Documentation of ADD/ADHD, Criteria for Documenting a Physical Disability, Criteria for Documenting a Psychological Disability
• If requesting alternative courses for the foreign language and/or the mathematics/logic requirement, write a letter to the Director of Disability Services stating the request. Submit this letter (Writing a Letter to Request Foreign Language and/or Math/Logic Alternatives) with the application form and documentation to SNAP Services.
• Optional: A recent photograph to help us remember you.

A letter of approval or denial will be sent to you in two to four weeks. If approved, make an appointment with a SNAP administrator to identify the services available to you. If denied, you may appeal to the Faculty Committee on Academic Standards.
