Absences

Below is a copy of the absence memorandum that the Office of Undergraduate Studies sends me to verify your absence from class:
Absence Memorandum

TO: Professor J Deavor
CHEM101001

FROM: Office of Undergraduate Studies

DATE: January 12, 2006

RE: Doe, John 109-99-999

Date and reason for absence indicated below:

1. January 12, 2006
2. Illness reported by Campus Wellness Center.

Please note that all this Absence Memorandum does is verify the documentation that you have provided the Office of Undergraduate Studies. It is not an excuse by which you are reprieved of responsibility from the class that you missed. You are still very much responsible for the content covered in class and all assignments that were made. It does not grant you the right to make up a test or quiz.

If you miss a quiz you will not be allowed to make it up.

If you know that you must miss a test, you must make arrangements with me prior to the time that the test is given. Call, email, fax, telegraph, speak to me in person, or send your roommate or "significant other" to let me know.
