Event Planning and Space Request Forms

- National Pan-Hellenic Council (NPHC) House Reservation Form (PDF)
Use this form to request use of the NPHC House located at 97 Wentworth Street. Forms must be submitted 2 weeks prior to the event. - Residence Life and Housing Room Reservation Request Form (PDF)
This form will allow you to request space in any of the residence hall programming spaces. You will receive a confirmation of your request by email if space is available and your event meets all guidelines. A reservation confirmation must be signed or replied to via email to finalize your event before the event date. - Student Classroom Reservation Request Form (PDF)
Only Registered/Sanctioned Student Organizations May Use College Facilities. No Computer Or Smart Rooms Are Available For Student Use. - Stern Student Center Room Requests
Completion of this form will allow you to request meeting space at the Stern Student Center. You will receive all correspondence, including confirmation of reservation, if the event is able to take place. - Student Life Event Planning Form
This form must be completed if you are planning an event on campus where alcohol will be served and/or public safety is needed for the event. PLEASE NOTE: this form must be completed 2 weeks prior to the event. - Domestic Travel Waiver Form
This form must be completed by your members if your organization is traveling off campus for an event.
